We are thrilled to share that Flemingdon Health Centre (FHC) has recognized our company with its 2025 Health System Champion award for exceptional leadership over the past years working on the Thorncliffe Park Community Hub.
Our team provided expert project management and operational readiness services to support the delivery and planned operation of the hub, which opened to Thorncliffe Park and Flemingdon Park residents and newcomers in February. Located in Toronto, Ontario, in one of Canada’s largest and most densely populated high-rise communities, the innovative multi-service space brings together health, wellness, social and support services under one roof to foster vibrant, healthy and connected communities.
Working closely with FHC and its community partners, our team project managed the redevelopment of the space, including coordinating requirements with multiple service providers such as Health Access Thorncliffe Park, The Neighbourhood Organization, Don Valley Community Legal Services, the Midwives’ Clinic of East York-Don Mills, Toronto Public Health, and others. We also helped advocate for additional funding needed to optimize the hub’s shared spaces. The hub is a true community gathering space that includes a large, well-equipped kitchen and a town square that serves as an ideal meeting place for cultural gatherings and celebrations.
Chosen from FHC staff nominations each year, the Health System Champion award celebrates organizations that support FHC’s vision for strong, healthy communities and work that has a positive, lasting impact in those communities. It highlights the value of collaboration between providers and project partners to create sustainable, community-focused facilities.
